Membership Rates

Individual Primary Care

  • Children 0-18 -$30/month with at least one parent enrollment *At this time we are not able to provide routine vaccinations, call to discuss how we can help arrange these for you.

  • Adults 20-50 - $60/month

  • Adults 51-64 - $85/month

  • Adults 65+ - $110/month

All new members will pay a one-time ENROLLMENT FEE of $100 at the first visit. You will NOT be billed today.

Regular billing will start the month you enroll regardless of the first visit date.

Primary Care for Business

  • Business owners can enroll at least 5 of their employees at only $60 per month per adult (regardless of age)

  • $30 per month per child (with a paying adult).

  • Under 5 employees, price will vary depending on age. Monthly fee is waived.

  • Please call for more information, (386) 227-7027. Enrollment fees are waived for corporate accounts with over 5 members.
