Membership Rates

Individual Primary Care

  • Children 0-18 -$30/month with at least one parent enrollment *At this time we are not able to provide routine vaccinations, call to discuss how we can help arrange these for you.

  • Adults - $110/month

All new members will pay a one-time ENROLLMENT FEE of $100 billed today.

Regular billing will start the after month you enroll regardless of the first visit date. You pay for January services in February.

Primary Care for Business

  • Business owners can enroll their employees at only $110 per month per adult.

  • $30 per month per child (with a paying adult).

  • Under 5 employees, enrollment fee is waived.

  • Please call for more information, (386) 227-7027. Enrollment fees are waived for corporate accounts with over 5 members.
