A Doctor of Nursing Practice (DNP), Adult-Gerontology Primary Care Nurse Practitioner (AGPCNP) is an advanced practice registered nurse (APRN) with extensive education and clinical experience. They provide comprehensive primary care services to adults and older adults, including diagnosing and treating acute and chronic illnesses, prescribing medications, and managing complex health conditions.

Our registered nurse assesses patient conditions, administers treatments, and provides essential health education. Additionally, RNs coordinate care, support chronic disease management, and advocate for patient needs, ensuring optimal well-being through preventative care and health promotion.

Medical Assistant
Our medical assistant handles administrative tasks such as scheduling appointments and managing patient records while also assisting with clinical duties like taking vital signs, preparing patients for exams, and administering medications as directed by the provider. Their contributions significantly enhance the efficiency and patient experience within the clinic.

Membership Rates
Primary Care for Individuals:
Children 0-18 - $30/month* with at least one parent enrollment.
At this time we are not able to provide routine vaccinations, call to discuss how we can help arrange these for you.
Adults 20-50 - $60/month
Adults 51-64 - $85/month
Adults 65+ - $110/month
All new members will pay a one-time enrollment fee of $100 at the first visit. You will NOT be billed today.
Regular billing will start the month you enroll regardless of the first visit date:
